How to Apply

Thank you for your interest in attending Hazel’s Heart Healthcare Academy Nurse Assistant Training Program. The following instructions provide information that will help you complete your application and ensure that it is processed in a timely manner.

APPLICATION REQUIREMENTS

All applicants must submit the following materials:

1. Application for Admission 

Please be sure to complete the enrollment agreement in its entirety. If you have any questions about its contents please do not hesitate to reach out.

 

2.  Registration fee of $75 (Non-Refundable)

The application fee is payable to Hazel’s Heart Healthcare Academy and can be paid in person or via invoice with credit/debit card. The application fee is not refundable.  This fee will be applied to your overall balance.

*If you qualify for any scholarships the registration fee is included so you do not have to pay an additional fee

3.  Documentation of Negative TB test or Chest X-Ray

TB screening (TB Skin Test or Blood Assay) is required of all students regardless of prior BCG vaccination.  All incoming students need to provide documentation of their TB status.

 

4.  Background Check

On May 14, 2002, the Governor of South Carolina signed the “Criminal Records Checks of Direct Care Staff” into law, effective July 1, 2002. This law requires that background checks be conducted on all individuals who provide direct patient care to residents of nursing homes, day care facilities for adults, home health agencies or community residential care facilities.

*Be sure to print background check immediately, once you leave the site you will not be able to review your result again.
